The Asbestos Project Permit Application Service allows you to apply for an asbestos project permit application or demolition notification.

NOTE! The submission of a project permit application or demolition notification does not automatically authorize you to conduct renovation or demolition activities. You must have a project permit or stamped demolition notification on site.

Special Notes
  1. If you are applying for an emergency renovation application or ordered demolition application, you may not use this service. You must first contact the Department of Environmental Quality Asbestos Control Program, at 406-444-5300 or email us at deqacponline@mt.gov, regarding the activity and you must submit the application on paper.

  2. Prior to conducting any demolition or renovation activity, the facility or part of the facility to be impacted must be inspected by a Montana-accredited asbestos inspector. This inspection must be kept on site during renovation and demolition activities.

  3. All fields are required to be filled unless marked "Optional."

To begin using this service, you must log in using ePass Montana, the state's single sign-on solution. ePass Montana allows you to use one username and password to access many eGovernment services.

Project Permit

If you are only enclosing, encapsulating, removing, repairing, or transporting and disposing of asbestos-containing waste (ACW) and there will be no further renovation or demolition activities, use this line of the service to apply for a permit to conduct these types of asbestos-related activities. For any others, please see the other options.

Demolition

If you demolishing a facility, facility component, or part of a facility and there is no asbestos-containing material in the building or the material was removed under a previous project permit, use this line of the service. If any asbestos-containing material will be left in the building (except for intentional burnings), you may not use this line of the service. You must use the Project Permit Application & Demolition Notification line of the service.

Project Permit Application & Demolition Notification

If you are enclosing, encapsulating, removing, repairing, or transporting and disposing of asbestos-containing waste, and also demolishing a facility, facility component, or part of a facility, use this line of service to apply for a permit to conduct these types of asbestos-related activities. You may use this form also if you are conducting enclosure, encapsulating, removing, repairing, or transporting and disposing of ACW only.

Inspection Information:

Prior to conducting any demolition or renovation activity, the facility or part of the facility to be impacted must be inspected by a Montana-accredited asbestos inspector. This inspection must be kept on site during renovation and demolition activities.

Permit Type:

  • Type of Activity: Select the type of activity to be conducted at the site. You may choose two.

Contractor:

  • Company or Contractor Name: Enter a company name or a contractor first and last name. Entering both will cause your application to err.
  • Mailing Information: Enter the mailing address for the contractor.
  • Contact Information: Enter the contact information including an email address. This email address will be used to correspond with the applicant and to whom the final permit will be sent.
  • On-Site Contractor Information: Select the name of the on-site contractor supervisor. This individual must be accredited during the dates of the project.

Site Info

  • Building Name/Site: Enter the name of the building or facility. If the project covers the demolition of two or more residential units, use "Demo Res NNNN Street Name" for the Building Name/Site. For example, “Demo Res 1720 Public St.” For other commercial or public buildings without names, some suggestions are “Commercial Bldg 1720” (1720 being the numerical portion of the street address), “Apartment Complex 1720.” Please note that if DEQ already has the location in its database, the name in DEQ’s database will be used.
  • Physical Street Address: The physical street address for the site. Do not enter a post office box.
  • City, State, Zip: City, State, and Zip for the site.
  • Building Size, in Square Feet: Enter a numerical number for the size of the building.
  • Number of Floors: Enter the number of floors. If the application is for a part of a facility, enter "1."
  • Age of Site, In Years: Enter a number for the age of the site.
  • Location Latitude and Location Longitude: Enter the latitude and longitude (lat/long) for the facility. You can click on the "Find your Lat/Long" button at the right to obtain lat/long information. This service will take you to a lat/long service. Follow the instructions on the screen. After you have chosen your site correctly, the service will automatically populate the lat/long parts for you.
  • Site Contact Person and Site Contact Telephone Number: Enter the First Name, Last Name, and Phone number of a site contact person that DEQ may contact during the dates of the asbestos project.
  • Site Building Owner: Enter the sites owner’s name.
  • Mailing Address: Enter a valid mailing address for the owner.
  • City, State, Zip: City, State, and Zip for the owner’s address.
  • Owner Contact Telephone Number: Enter a contact number for the owner.
  • Present Use: Choose the option that best describes the present use of the facility. You may choose two.
  • Prior Use: Choose the option that best describes the prior use of the facility. You may choose two.

Dates:

  • Scheduled Dates for Asbestos Activity: The start date must be at least 10 working days from the date of submission.
    1. Start Date: Enter the date asbestos-related activities will be begin on the site. This is the date when asbestos-containing materials will first be impacted.
    2. Complete Date: Enter the date asbestos-related activities will cease on the site. This should be the day that final visual and air clearances will be accomplished.
  • Scheduled Dates for Asbestos Waste Disposal: Use the dates to notify DEQ of later dates when the ACW will be disposed of when not being disposed of during the permit dates. The start date must be at least 10 working days from the date of submission.
    1. Start Date: Enter the first date expected to transport and dispose of ACW at an approved Class II or IV landfill.
    2. Complete Date: Enter the last date expected to transport and dispose of ACW at an approved Class II or IV landfill.

Approximate Amount and Type of Asbestos Material:

  • Regulated Material:
    • Amount: Enter a figure for the amount of the material to be removed
    • Measurement: Select a measurement.
    • Material Type: Select a material type. You must list at least one regulated asbestos-containing material.
    • Regulated ACM: Select the material.
  • Non-regulated Material:
    • Amount: Enter a figure for the amount of the material to be removed
    • Measurement: Select a measurement.
    • Material Type: Select Non-friable ACM to be removed. After this is selected, Cat I or Cat II must be completed.
    • Regulated ACM: Select the material.

Click the "Add Material" button when done with the specific material. You may add up to 25 materials on this application. If you have more than 25 materials, please contact DEQ at (406) 444-5300.

Design:

  • Description of planned demolition or renovation work and method(s) to be used: Enter a description of the planned demolition or renovation work and the method(s) to be used. If a demolition is being notified with the application, the planned demolition work methods must also be listed.
  • Description of work practices and engineering controls to be used to prevent emissions of asbestos at the demolition and renovation site: Describe what will be done to prevent emission of asbestos and dust at the site in 10 or more words. If the asbestos contractor has a Standard Operating Project Design filed with DEQ, check the "Optional" box and list the project design number.
  • Descriptions of procedures to be followed in the event that unexpected asbestos is found or previously non-friable asbestos material becomes crumbled, pulverized, or reduced to powder: Describe the procedures to be followed when unexpected asbestos is found or previously non-friable asbestos material becomes regulated.
  • Name of Project Designer: Select the name of the project designer for the project.

Transporter:

  • ACM Waste Transporter Information: If the transporter is the same as the asbestos contractor, check the box next to "Check if same as Asbestos Project Contractor." Otherwise, entered the requested information.
  • Enter information for the Transporter contact.

Disposal:

Select a disposal site. If "Other" is chosen, complete the requested information.

Signature:

  • Name: Type the first and last name of the person signing the application.
  • Date: Enter the date the application is being signed/submitted.

Fees:

  • Contract Volume Cost: Enter the Contract Volume Cost for the project. The permit fee will automatically calculate as the amount is entered.
  • Non-Associated Costs: Enter any non-associated costs with the project.

Application Summary:

This is a summary of your application that will be submitted. Review each section for accuracy. You may edit any section before submittal.

If everything is correct, print a copy of your application to be posted on site. Then click the Next button.

Payment

  • Select a Payment Option: Select a payment option for how fees are to be paid.
    • Credit Card - Press this button to pay using a Visa, MasterCard, or American Express.
    • Electronic Check - Press this button to pay using either a checking or savings account.

Pay with a Credit Card

The top section is for collecting billing information. This information should correspond with the credit card being used.

Once all of your information is entered, press the "Submit for Purchase" button. Please only press this button once. Pressing this button multiple times may result in multiple payments.

If you wish to use a different payment method, press the "Back" button.

Pay with an eCheck

The top section is for collecting billing information. This information should correspond with the bank account being used. The "Bank Account Number" and "Verify Bank Account Number" fields must be the same.

Once all of your information is entered, press the "Submit for Purchase" button. Please only press this button once. Pressing this button multiple times may result in multiple payments.

If you wish to use a different payment method, press the "Back" button.

Receipt

This page contains the Payment Date and time, a Unique ID for the transaction, and the Amount Paid. Please print this page for your records.

If desired, you may Print the application, View Permit, Complete a New Application, or Log Out of the service

Approval

Once you have paid for your project permit/demolition notification application, DEQ must review it before approving the application and issuing a permit. If there is a problem or more information is needed, DEQ will contact you. DEQ will make decisions on applications with a contract value of $3,000 or less within seven calendar days. Approval of an application with a contract value greater than $3,000 can sometimes take up to 10 working days.
